We have recently undertaken a restructuring of our organisation to ensure that we are fit for purpose to deliver the best outcomes for people who use our services in an ever more challenging operating environment. We have introduced this post as part of this process.
Working as part of a team of six Team Leaders, you will have responsibility for the day to day management of our community support provision, ensuring that it provides an extensive range of personal and social support to people with a range of care and support needs. You will ensure that the service provided operates in accordance with internal quality frameworks, external regulatory standards and best practice principles. The role has responsibility for the management of referrals to the service and the provision of line management for support staff. We provide flexible support tailored to individual needs and wishes provided in peoples own homes, within their communities and from our community centres.
You will need knowledge of best practice in the delivery of community based care and support services to adults with a range of needs (e.g. learning disability, mental health, physical and sensory needs, older people, dementia, complex needs). You will have significant experience of working within a supervisory role within a social care setting and hold a relevant professional qualification, such as Level 3 Diploma in Health and Social Care, or be working towards this.
You will be an effective leader able to motivate and engage staff to promote positive outcomes, quality and continuous improvement. You will also need a sound understanding of person-centred approaches in care and support planning.
